Common Single Need Referral Form (CSNRF)
The Common Single Need Referral Form (CSNRF) will be launched on 1st April to be used when a single need has been identified for referrals into the Children and Young People's Disability Partnership.
Please Note: The Common Single Need Referral Form is for use only for the Disability Partnership during the trial period of 1st April to September.
Following on from a review of the use of CAF within the partnership, including feedback from partners, we are now introducing the common single need referral form (CSNRF) into the suite of common processes.
This form is to be used when a single need is identified that a service within the CYPDisP can meet. The CSNRF can then be completed to refer that child to the service. The partnership will not accept any other referral format.
If there is a CSNRF already completed for the identified child, then a CAF needs to be completed.
