If your child is currently going through the Statutory Assessment process or has a statement of special educational needs these are the Parental forms/Booklets we send out at different stages.

They include:

Appendix A form
This form you’ll receive in the first instance from us whether you are just starting S/A or if you have applied for a Parental Referral.

We’ll also send you a pack which includes the following:

Proposed Statement form
When your child’s statement is written and support is agreed at Panel, this form is sent with a copy of the statement and a pack which contains the following:

Parental Questionnaire
When your child’s statement is first finalised, this questionnaire is sent out to parents/carers for your opinion on our service. This questionnaire is not mandatory but your opinion does help us improve our service to you.

Proposed Amended Statement form
When your child’s statement is finalised, this form that is sent if your child is transferring schools and also if there are any amendments to your child’s statement.

Who is the ARO for my child’s school?
The ARO’s are allocated to different schools within the Stockport area, click here to see which ARO has been assigned to your child’s school.
